Things to Bring to Your Appointment
Please note: A 15-minute grace period is allowed for late arrivals. Appointments will be rescheduled if imaging discs or required documents are missing.
To help us provide the best care, please arrive a few minutes early and bring the following:

CD disc or copies of any prior imaging (MRI, CT scans, etc.)

Insurance cards

Driver’s license or valid photo ID

List of current medications and dosages

Surgical and medical history

Referral form (if applicable)

Any additional documentation from your referring provider
